Kelvale can help you manage your longer term health issues such as:

  • Diabetes
  • Asthma
  • Chronic lung disease
  • Cardiovascular (Heart) disease
  • Osteoporosis
  • Osteoarthritis
  • Weight mangement

And other long term conditions including people who have a disability.

We have dedicated primary health nurses who will regularly send you reminders for your pathology tests, nurse and doctor visits and referrals to relevant other specialist providers.

In this way, nothing is overlooked and you will have the best management of your condition.

Assessments:

You will be assessed by our primary health nurse and your doctor.

A care plan will be formulated for you, outlining your treatment goals and management.

This can then include Medicare funded access through team care arrangements to:

  • Physiotherapists
  • Podiatrists
  • Exercise physiologist
  • Dieticians
  • Others as approved by Medicare

This will enable you to receive Medicare rebates for these services.

Appointments:

One hour appointments comprise consultation with our Primary Health nurse followed by your doctor.

You will first see the nurse on arrival for your assessment, followed by a visit with your doctor to review your Care Plan.

Care Plans and Team Care arrangements known as Enhanced Primary Care plans (EPCs) will be bulk-billed for Pensioners and children under 16.
